Additional copies can be picked up in the church office or entry to the church OR your entire family can use the same form and initial the blanks that pertain to each family member. This form can be accessed from our church website, welcometopeace.org, and can be submitted via email, OR the form can be dropped off in the church office or drop box outside the office or church.
